Have you heard about the NHS Primary Care Networks?

All GP practices across the country will come together into networks covering around 30,000 to 50,000 patients.

In the very south of Buckinghamshire, surrounding Slough, the network starts in July 2019 and will consist of

• Burnham Health Centre

• Southmead, Farnham Common.

• Threeways, Stoke Poges

• Iver Health Centre

• Denham Medical Centre

They will eventually deliver services to improve

• Prescription reviews for people with many medications

• Health care for residents in care homes

• Crisis management of people with long term conditions

• Health and wellbeing advice and support to prevent long term conditions

• Diagnosis of early cancer

• Early support for people with cardiovascular disease

• Local action to tackle health inequalities

PCNs will work with a wider range of staff than are currently available in most surgeries – social prescriber link workers, clinical pharmacists, first contact physiotherapists, physicians’ associates and paramedics. They will also be working far more closely with local community teams, adult social acre and others such as charities.
